Please bring with you on your first visit the following:
- Insurance Cards
- All eye drops that you are currently using, including non-prescription drops
- List of your Medications and Allergies
- List of your prior ocular history, prior medical history, and prior surgeries
- Completed Intake Forms sent to you prior to your visit
- Valid Referral, if required by your Insurance Company
- Name, address, phone # of your PCP (Primary Care Physician) and person that referred you to our office
- Emergency Contact Information, including your contact’s cell phone number.
- Name, address, and phone # of your pharmacy.
